By making a purchase with Erickson Dermatology, you are confirming that you have read and understand this no refunds policy. Skincare products are non-returnable; if the product is damaged, an exchange for the same product or an account credit will be provided.
Payment is due in full at the time services are rendered. We accept Cash, Debit Cards, Visa, MasterCard, American Express, Discover, Apple Pay and Care Credit. Personal checks are not accepted.
All patients new to Erickson Dermatology will require a cosmetic consultation prior to receiving any services or treatments. For cosmetic consultation appointments, a non-refundable consultation deposit of $250 is due at time of booking. This deposit will be applied toward any services or treatments you elect to schedule and complete in our clinic within 30 days of your consultation. If you elect to not schedule any services or treatments with us within 30 days of your consultation, the $250 consultation deposit (or any balance remaining from this $250) will be applied to cover the cost of the consultation itself. This consultation fee cannot be refunded nor submitted toward insurance. A cancellation or re-scheduling of this cosmetic consultation within 48 hours of the scheduled appointment time will result in a forfeiture of the $250 deposit made and will require another $250 deposit to re-schedule. However, when re-scheduling >48 hours in advance of your appointment, your deposit will carry forward to your re-scheduled consultation appointment.
All appointments that are scheduled for services or treatments lasting one hour or longer in duration will require a 20% advance-deposit based on the total treatment cost. Same day no-shows or cancellations within 48 hours of the appointment time** will result in forfeiture of the deposit made and will require another deposit to re-reschedule the appointment. Similarly, you will be deemed a same day no-show if you arrive for your appointment beyond 15 minutes of your scheduled appointment time. No-shows, late arrivals and cancellations cause disruptions to our schedule and compromise the experience and level of service that all of our clients deserve. Providing us with ample cancellation/re-scheduling notice (> 48 hours)** will not result in a forfeiture of the deposit made as it allows us the opportunity to offer that appointment time to other clients who are seeking near-term appointments.
For appointments where a deposit is not required, a $150 cancellation fee will be assessed to same day no-shows or cancellations within 48 hours of the appointment time**.
** Please note that any Monday appointments (or Tuesday appointments when the office is closed on Monday) must be cancelled by 3pm on the preceding Thursday to avoid a forfeiture of the deposit made or the cancellation fee.
Dr. Erickson is a Board-Certified dermatologist and a fellowship-trained Mohs surgeon. Although insurance IS NOT accepted as most of the services rendered in this office are not covered by insurance, we do provide needed information for certain procedures where you are able to submit to your insurance, if you choose, for reimbursement. We are considered out-of-network by insurance providers and as such, make no assurance of reimbursement.
In addition to the cosmetic services Dr. Erickson provides, she also provides skin exams, performs biopsies and treats general dermatology concerns for her patients. In the circumstance where a biopsy or laboratory specimen is submitted to an outside lab, a copy of the patient’s insurance card and a photo ID will be made and submitted to the lab with the specimen so that the lab may bill your insurance. If your insurance does not cover or only partially covers the cost of the lab(s), you will receive a bill directly from the lab that will be your responsibility to pay. Any fee(s) charged by the lab will be in addition to the in-office fee for the procedure(s) associated with obtaining the specimen. Alternatively, we do have cash pay rates established should you prefer this option.
